Please note:
- If a Food vendor, participants agree to follow Allen County Board of Health, Indiana State Board of Health, and CDC Guidelines
- Vendor participation is subject to approval and is not guaranteed you will receive an email confirmation
- All participants are provided a 6-Feet Table OR 10x10 Foot Tent upon registration, participants are allowed to bring their own equipment.
- Each vendor must display a sign identifying the name of the business
- Vendors are required to know and have appropriate labeling and/ or permits required.
- Vendors agree by this application form to accept the following hold harmless clause:

All authorized vendors participating in any MONO Events agree that they are independent operators and shall be individually liable for any loss, personal injury, deaths, and/ or any other damages that may occur as a result of the vendors negligence or that of its employees, agents, or associates. All vendors agree to indemnify and MONO Customs Solutions LLC, and their employees and agents harmless from any loss, costs, damages, and other expenses, including attorney's fees, suffered or incurred by MONO Custom Solutions LLC, their employees & agents by reason of the vendor's negligence or intentional misconduct or that of its employees, agents, and associates. It is recommended that each vendor carry his or her own and product liability insurance
